2017 Volunteer Application

2017 returning volunteer form

Bangor West is required by Little League International to conduct background checks on all of our volunteers through First Advantage.

If you are a new volunteer to Bangor West Little League, please complete the 2017 Volunteer Application. The Volunteer Application can be emailed to me at [email protected] or mailed to 30 Fowle Road, Bangor. Please note that your social security number is required for the background check. Once the background check has been completed, your social security number will be redacted on the form and placed in my volunteer application file. (I am the only person that will have access to your personal information.)

If you are a returning volunteer to Bangor West Little League, please complete the 2017 Returning Volunteer Application and email or mail it to me.

All coaches, assistant coaches, parents assisting with a team or volunteering at concessions should complete a volunteer form for our file.
